lDocumentation of controlled substances dispensed from office stock must be maintained separate from patient charts
lIt is highly recommended that maintenance of documentation of controlled substances administered in the office be kept separate from patient charts
lRequired information includes:
–Date
–Patient name and
species
–Owner name and
address
–Drug name, strength, dosage form, quantity and initials of staff administering or dispensing
