Receipt Records – Schedules III-V
lYou must maintain a record of all controlled substances received
lReceipt records must contain:
–Supplier’s name, address and DEA number
–Receiver’s name, address and DEA number
–Drug name, strength, dosage form and quantity
–Date received – If you use invoices as receipt records, you must note the date received (not date shipped) on the invoice.
