Registration Issues
lThe Bureau must be notified of a change in practice location within 30 days of the change, or the registration automatically terminates
lSeparate registrations are required for separate locations where controlled substances are stored.
lLocum Tenen (LT) registrations must use the same address as on the professional license.  LTs may not stock controlled substances. They may only work at one site 90 days in a row and must keep a log of locations where they have worked
